The LJBD/CWDS brand is owned and operated by Chris Williams whose registered office is 91 Lake Road, Lake Jackson, TX 77566. General Lake Jackson Ballroom Dancing/Chris Williams Dance Studio strongly recommends that you consult with your doctor/GP before beginning any dance class (studio or online) if you have any medical concerns. You should be in good physical condition and be able to participate in the class. LJBD/CWDS is not a medical professional and therefore has no expertise in diagnosing, examining, or treating medical conditions of any kind, or in determining the effect the classes will have on a medical condition. Adult I am aware that participation in a dance class at the studio requires physical exertion and involves the risk of injury. I understand these risks may include, but are not limited to injury to ligaments, muscles, tendons, bones, and other aspects of the body that may include head, neck, or spine. I understand that it is my responsibility to ensure that I am able to participate in the activities provided by LJBD/CWDS. By accepting this form, you agree to release by LJBD/CWDS and its employees, contractors or agents from any and all liability/responsibility which may arise in connection with your participation in activities at by LJBD/CWDS. Imagery We believe that celebrating achievements within our dance school is an important part of the learning and personal development experience. Taking photographs/videos of students during their training and to celebrate individual and group successes is a valuable way of building individual confidence and a strong studio community. We use photos and videos for a variety of dance related reasons. These include; as a useful tool for the recognition of progression, achievement and celebration of various aspects of dance training, including, class, performance, competitions and the promotion of the work undertaken by LJBD/CWDS. The imagery we take may be used on the official by LJBD/CWDS website, social media pages, brochures & other advertisements. By signing this form, I understand that I am giving unrestricted permission for my/my child’s image to be used in print, video, and digital media by LJBD/CWDS. Cancellation Policy

Cancellations must be made 24 hours prior to your scheduled pre-paid private lesson time. Failure to cancel without a 24-hour notice will be charged as (1) lesson with the same applying to a no show. Rescheduling policy (Effective 10/13/2023) - You are allowed (1) reschedule per 5-lesson package or (2) reschedules per 10-lesson package. Rescheduling must be done 24-hours prior to your lesson time. Rescheduling more than the adherence of your package will result in a 1 lesson forfeiture per lesson.
